Floater Position to work in Boulder

Host Home Provider - Longmont


Floater Position: We are currently accepting applications for a position that will work in both the Employment Access program and the Senior Opportunity Source program.

Employment Access Coordinators work with a group of four to five individuals in the community. Activities may include volunteer work, paid work, or vocational skill development. Volunteer work is the most common type of activity.

Senior Opportunity Source Community Connectors assist adults of retirement age with developmental disabilities in accessing the community for activities and forming relationships within the community. The Community Connector is responsible to provide a variety of senior related services in the community with the individuals we serve in small groups.

This is an entry level position. The work week is Monday through Friday, 7:45 a.m. to 3:30 p.m. A B.A. in social science preferred, as well as 3 months experience working with persons from a variety of social, cultural, economic and educational background. A valid Colorado driver's license is required. The job involves transporting individuals in agency vans. Physical demands for the job include lifting (60 lbs. or more, associated with consumer transfer); pushing, pulling, stooping, bending, twisting, reaching, turning (preceding demands primarily associated with tying down wheelchairs); driving; sitting; ambulating, and computer use. The position pays $10.00 per hour. Required Documentation: Driver’s license with less than 3 moving violations or 1 serious moving violation (as defined in the MVR Procedure), and consent for Colorado Bureau of Investigation and MVR check.

Host Home Provider: Our company is dedicated to providing quality services for people with developmental disabilities. Host Home Providers open their homes to individual(s) with developmental disabilities and provide a variety of supports to meet the individualized needs of the people we serve. Successful candidates will be compassionate people who want to make a difference in the lives of others.

A critical component of our hiring process is matching the provider to the needs and preferences of the person receiving services. We look carefully at the applicant’s qualifications, personal compatibility with the person, and environmental characteristics such as location, potential roommates, presence or absence of pets, and other individualized matters.

Because of this, an applicant may be a very good provider, but something may not be the best match for the person. If you are not selected the first time you apply, we would like to keep your application on file, ready to be considered for the next person requesting services.

We are currently hiring a provider for a young adult woman. She has many of the skills needed to live independently, but needs someone there to continue to encourage her personal growth and emotional development. Provider will be responsible for light housekeeping, administering medication, assisting with meal preparation, connecting her with community resources, providing transportation to and from appointments, training and support around individual goals/objectives, maintaining documentation of supports and services, and serving as a liaison between her and other members of her team.

Experience preferred, but we will provide training for the right candidate. We continue to provide ongoing training and support for providers and the people we serve to insure successful placements.

Preferred Qualifications:

-Home in Longmont
-Female provider
-No children

Minimum Requirements for all Host Home Providers:

-Complete a Host Home Provider Application
-Successfully pass MVR and background check
-Provide a home that meets HUD standards
-Ability to become CPR, First Aid, and Medication Administration certified
-Demonstrate good interpersonal skills and knowledge of basic training techniques
