Community Connector Host Home / Live-In Provider Ongoing
Community Connector
Community Connectors start their day at 7:45 a.m. at the Community Link office. They then proceed to pick up the individuals they will be working with by using a company minivan. From 9:00 a.m. - 2:00 p.m., Community Connectors work with a group of four to five individuals in the community. Activities may include volunteer work, paid work, or vocational skill development. Volunteer work is the most common type of activity. Some of the non-profits in the county where volunteer work is performed include Meals on Wheels, Community Food Share, Elder Share, People's Clinic, and many more. Community Connectors provide instruction, supervision, safety monitoring and assistance to individuals in their group. At 2:00 p.m., Community Connectors transport their group members home and return to the office to complete light paperwork. The work day ends at 3:30 p.m. All activities and transportation occur in Boulder County.
The Employment Access Coordinator position is an entry level position that does not require specific experience. A BA in Social Science is preferred and a valid Colorado driver's license is required. Physical demands for the job include lifting (60 lbs. or more, associated with consumer transfer); pushing, pulling, stooping, bending, twisting, reaching, turning (preceding demands primarily associated with tying down wheelchairs); driving; sitting; ambulating, and computer use. The position pays $10.00 per hour and is a 37.5 hour work week. Required Documentation: Driver’s license with less than 3 moving violations or 1 serious moving violation (as defined in the MVR Procedure), and consent for Colorado Bureau of Investigation and MVR check. Host Home / Live-In Provider:
We are looking to hire fun and compassionate people to support individuals in their homes. We currently have one opening in Boulder County. Please note that being a host home / live-in provider is not shift work. These positions are modeled after Foster Care Programs.
Position 1: Individual can be very independent and has come extremely far in a short period of time with consistent, positive support. She has a wonderful personality and can be very thoughtful. She really enjoys exercising and eating well, so this should also be a priority of her host home provider. Assistance is required with medical matters (appointments, medication administration, positive encouragement). Assistance is also needed to help create and maintain a regular schedule of activities balanced with relaxation time. Individual works and attends day program during the week, and also enjoys swimming, artwork, softball, yoga, etc.

Position 2: This individual likes spending time with others and helping out around the house. He is usually up for any activity suggested, but especially enjoys going for walks, playing softball, horseback riding, swimming, and bbqs with friends/family. He lots of independent living skills, but needs daily support to maintain a healthy diet, administer medications, and assistance to schedule and attend community activities. Additionally, he receives support from a Behavioral Therapist and benefits from HHP providing ongoing education and support to practice therapist’s recommendations throughout the week in order to continue increasing his current skills. He attends a day program 5 days a week and enjoys participating in activities through the local recreation centers.
